John (Samuel Hunt) Dixwell M.D., Physician, of Boston, Massachusetts

The Massachusetts Historical Society reports that in 1804, Samuel Hunt, taking his mother's maiden surname and altering his given name as well," changed his name to John Dixwell to avoid confusion with another Boston physician named Samuel Hunt."
